Chronic Disease Management in Coorparoo
Living with a chronic health condition can affect every aspect of your life. The right support — with clear goals, coordinated care, and a GP who knows your history — can make a significant difference to how you manage day to day. At Coorparoo Clinic, our GPs work with you to develop a personalised approach to managing your long-term health.
We provide care for a wide range of chronic conditions, including asthma, cardiovascular disease, type 2 diabetes, musculoskeletal conditions, stroke recovery, and cancer follow-up.
GP Management Plans and Team Care Arrangements
Medicare funds two key planning tools for patients with chronic conditions:
GP Management Plan (GPMP) — your GP prepares an individual management plan that sets out your health goals, the actions you and your GP will take, and any services you may need. A GPMP can provide you with up to five Medicare-rebated allied health visits per year (for services such as physiotherapy, podiatry, dietetics, and psychology).
Team Care Arrangement (TCA) — when your care involves three or more health providers (for example, your GP, a physiotherapist, and a diabetes educator), a TCA formalises the collaboration and coordinates your care. A TCA must be in place for you to access the five allied health visits.
Both a GPMP and a TCA require a longer appointment. Please book accordingly, and let reception know the reason for your appointment so that the right amount of time is allocated.
Condition-specific care
Diabetes — blood glucose management, HbA1c monitoring, foot care, eye checks, and coordination with endocrinologists and diabetes educators where appropriate.
Cardiovascular disease — blood pressure management, cholesterol monitoring, medication reviews, lifestyle advice, and coordination with cardiologists where needed.
Asthma and respiratory conditions — inhaler technique review, personalised action plans, spirometry where indicated, and regular review of symptom control.
Musculoskeletal conditions — chronic back pain, osteoarthritis, and other musculoskeletal concerns can be managed with a combination of GP-directed care, physiotherapy referral, and where appropriate, specialist input.
Stroke recovery and ongoing neurological care — follow-up care post-stroke, medication management, and coordination with rehabilitation and specialist services.
What to expect
Managing a chronic condition is an ongoing process, not a one-off consultation. Your GP will work with you to set realistic health goals and schedule regular review appointments to check on your progress, adjust your management plan if needed, and renew referrals.
It helps to keep a list of your current medications and any recent investigation results. If you are seeing other health professionals as part of your care, bring their reports or letters so your GP can keep the full picture.
